29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Production Floor; On or about October 5, 2016 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to work inside the #2 Size and Fit Machine Cell to perform a parts change over without locking it out in accordance with the established procedures. b) Production Floor; On or about October 14, 2016 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to work inside the #2 Size and Fit Machine Cell to perform a parts change over without locking it out in accordance with the established procedures. c) Production Floor; On or about October 14, 2016 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to work inside the AN CTR PE2 Robotic Cell to clean without locking it out in accordance with the established procedures. d) Production Floor; On or about October 17, 2016 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to work inside the AN CTR PE2 Robotic Cell to clean without locking it out in accordance with the established procedures.
