29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) AC Exteriors L.L.C. at 3545 S Oneida St, Denver, CO 80211: On and before November 10th, 2016, the employer did not ensure that each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was protected from falling.  Employees were shingling roofs with up to a 5 on 12 pitch roof, and were working without fall protection.  This condition exposed the employees to a fall hazard of up to approximately 16 feet.
