29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) when the operator had been involved in an accident or near-miss incident to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely: A) Pioneer Forge, Division of Powers & Sons, LLC, located at 101 Industrial Avenue, Pioneer, OH 43554: On or about November 9, 2016, the employer did not provide refresher training in safe operation of sit-down powered industrial vehicles for the employee who was involved in an incident while driving the Crown forklift (Serial # 9A172928) in Cold Work Department. The operator caught and disconnected one of the Possons pull-back safety device cables. Employees were exposed to struck-by hazards due to the lack of refresher training.

29 CFR 1910.1030(g)(2)(i): The employer did not train each employee with occupational exposure in accordance with the requirements of this section: A) Pioneer Forge, Division of Powers & Sons, LLC, located at 101 Industrial Avenue, Pioneer, OH 43554: On or about November 9, 2016, the employer did not ensure the each employee with occupational exposure to bloodborne hazards was trained in accordance with the 29 CFR 1910.1030 standard. Such training should have taken place at the time of the employees initial assignment, and should have covered items such as, but not limited to, a copy of the regulatory text of the 1910.1030 standard, explanations of the modes of transmission of bloodborne pathogens, and a general explanation of the epidemiology and symptoms of bloodborne diseases.
