Employees did not control all hazardous energy sources such as, but not limited to, electrical prior to conducting activities such as, but not limited to, un-jamming the conveyor. Employees were exposed to struck-by and caught-in-between hazards while performing this task.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    A)  Pioneer Forge, Division of Powers & Sons, LLC, located at 101 Industrial Avenue, Pioneer, OH 43554: On or about March 17, 2020, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not de-energize the Wardcraft Conveyor (Model: 5HSB1600-T1-41; S/N 45463-001-2/12), located next to the Saw 2 Shear in the Shear Department, prior to performing activities such as, but not limited to, un-jamming the conveyor. Employees were exposed to struck-by and caught-in-between hazards while performing this task.      As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from struck-by and caught-in-between hazards while employees conducted activities such as, but not limited to, un-jamming the Wardcraft Conveyor.
