29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Building 2: On or about November 20, 2016, and at times prior, two workers were exposed to falling thirteen feet to the deck below where a fall protection system was not used near an unguarded deck edge.    b) Building 5: On or about November 20, 2016, and at times prior, a worker was exposed to falling nineteen feet to the floor below where a fall protection system was not used near the unprotected edge of a stairway landing.  The employer corrected the condition during the inspection.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a) Building 2: On or about November 20, 2016, and at times prior, two workers were exposed to hazards while ascending and descending an elevation difference of three feet without a ramp, ladder of other means of safe access.  The employer corrected the condition during the inspection.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, and the ladder was secured at its top to a rigid support that will not deflect, a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a) Building 2: On or about November 20, 2016, and at times prior, two workers were exposed to hazards while transitioning between a stepladder and the upper level where a grab bar was not available above the ladder to provide hand holds while moving between the upper landing level and the ladder.  The employer corrected the condition during the inspection.
