29 CFR 1910.421(a): The employer did not comply with the requirements of paragraphs (b) through (h) of this section prior to each diving operation: Drake's Estero: On September 14, 2016, the employer failed to provide the necessary elements of a Pre-dive Procedure which included; emergency procedures, an assessment of the dive operation, and the use of a warning signal (an A Flag). An employee performing commercial diving activities was exposed to suffocation hazards.
