1926.601(b)(14) All vehicles in use was not checked at the beginning of each shift to assure that all parts and accessories were in safe operating condition and free of apparent damage that could cause failure while in use. All defects were not corrected before motor vehicle was placed in service. (a) Marine Operation Site, Point Reyes National Seashore, Drakes Estero: The employer operating Mini Bob Cat excavator aboard a floating barge did not check a mini Bob Cat excavator prior to using it to move materials aboard a floating barge. Employees were exposed to struck or run over by the equipment.

1926.602(a)(9)(i) All bidirectional machines, such as rollers, compacters, front-end loaders, bulldozers, and similar equipment, was not equipped with a horn, distinguishable from the surrounding noise level, which shall be operated as needed when the machine is moving in either direction. The horn was not maintained in an operative condition. (a) Marine Operation Site, Point Reyes National Seashore, Drakes Estero: The employer operating Mini Bob Cat excavator aboard a floating barge did not have a working horn. Employees were exposed to struck or run over by the equipment.
