29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:    Workers were observed engaged in residential construction activities building a four unit housing structure. Workers were exposed to potential electrocution hazards in that there was a yellow electrical extension cord connected from a GFCI outlet to a DeWalt Cutoff Saw which was installed on the ground across the road exposed to vehicular traffic.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.        Workers were observed engaged in residential construction activities installing wooden framing material for a 4 unit housing structure. Workers were exposed to 11.8 foot fall hazards in that the ladder was not 3.0 feet above the roof landing area.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.        Workers were engaged in residential construction activities building a multi-unit housing structure. Workers utilized a portable ladder to access and egress the upper level of the roof. Workers were exposed to potential fall hazards in that the workers were not properly trained in ladder safety requirements.

29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.      Workers were observed engaged in residential construction activity framing a 4 unit housing structure. Workers were exposed to potential fall hazards between 11.8 feet and greater than 25.0 feet fall hazards in that frequent and regular inspections of the jobsite, materials, and equipment was not conducted by a competent person to ensure the jobsite, materials, and equipment such as fall protection systems were maintained and utilized in a safe manner.      The Super Builders, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 900864, citation number 1, item number 1 and was affirmed as a final order on July 10, 2013 with respect to a workplace located at 9717 Crooked Creek Drive, Centerville, Ohio.      The Super Builders, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1099757, citation number 1, item number 1 and was affirmed as a final order on January 25, 1916 with respect to a workplace located at 1600 Cedar Creek Dr., Dayton, Ohio.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.       Workers were observed engaged in residential construction activities installing wooden framing material for a four unit housing structure. Workers were exposed to potential fall hazards between 11.8 feet and greater than 25.0 feet in that a proper fall protection system was not utilized.        The Super Builders, LLC Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 900864, citation number 1, item number 2 and was affirmed as a final order on July 10, 2013 with respect to a workplace located at 9717 Crooked Creek Drive, Centerville, Ohio.
