The employees were framing a newly constructed residential home located in Medina, Ohio 44256. There was one employee working on the roof under two other employees without wearing a protective helmet.

On or about November 18, 2016, employees were exposed to fall of approximately 9 feet. The employees were framing a newly constructed residential home located in Medina, Ohio 44256. The employees were using a ladder to assess the roof through a hole in the sheathing. The ladder did not extend at least 3 feet above the landing area.
