The employer is failing to protect employees from caught in and amputation hazards. This was most recently documented at the establishment located at 15 SW. 40 Ave in Great Bend KS, in that machine specific energy control producers were not developed, documented or utilized for the following pieces of equipment:  a) The SleevCo machine at the F-Line, utilizing electrical, pneumatics and liquids  b) The Oden Filling machine at the Cube-Line, utilizing electrical, pneumatics and liquids  c) On or about 11/17/16, an employee engaged in production activities at the Bekum Blow Molding machine in the plastics department was exposed to struck-by hazards in that the machine specific energy control procedures were not used.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The employer is failing to protect employees from amputation, crushing and struck-by hazards. This was most recently documented on or about 11/17/16 at the establishment located at 15 SW. 40 Ave in Great Bend KS;  a) On or about 11/17/16, an employee engaged in production activities at the Bekum Blow Molding Machine in the Plastics area was exposed to struck-by hazards in that the interlocks for the guards were secured in an inoperable position with zip ties.    b) The Atlas drill press (Property #01998) in the maintenance shop was not guarded to prevent contact with flying chips and debris and contact with the rotating chuck.    c) The Central Machinery drill press (SN- 9980 Model T-2119/9970) in the maintenance shop was not guarded to prevent contact with flying chips and debris and contact with the rotating chuck.    d) The Walker Turner drill press (Property #01928) in the machine shop was not guarded to prevent contact with flying chips and debris and contact with the rotating chuck.    e) The Conveyor belt at the south end of the Bekum plastic molding machine in the plastics area was not equipped with a guard which covered the top in-running nip point.  f) The Hytrol Conveyor (SN- #302844) at the F-line was not equipped with a guard which covered the top in-running nip point.
