29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  a) On or about November 21, 2016- job site of 75-76 180th Street, Fresh Meadows, NY 11366  an employee was using a Makita GA 7021 grinder, to cut concrete blocks, while not wearing eye protection.  WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PERSUANT TO 29 CFR 1903.19    b) On or about November 21, 2016- job site of 75-76 180th Street, Fresh Meadows, NY 11366  employees were mixing and applying Type I-II Portland Cement, a corrosive, while not wearing eye protection.  WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PERSUANT TO 29 CFR 1903.19

WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PERSUANT TO 29 CFR 1903.19
