29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation: a. On or about November 21, 2016, at the above addressed jobsite, employees were performing masonry work from a tubular welded frame scaffold, sixteen (16) feet one (1) inch above the ground, that was erected with unstable mudsills and baseplates, or with missing baseplates, thereby exposing employees to falls.

On or about November 21, 2016, at the above addressed jobsite, employees performing masonry operations from a tubular welded frame scaffold, sixteen (16) feet one (1) inch above the ground, were exposed to injuries from falls due to the lack of guardrails.
