On or about November 17, 2016 and at times prior thereto, at the worksite located at 801 N. Jefferson Street, Springdale, Arkansas, the company lock out/tag out program did not clearly and specifically outline the scope, purpose authorization and rules for energy isolation. On or about October 31, 2016 and at times prior thereto, at the worksite located at 801 N. Jefferson Street, Springdale, Arkansas, a moulding machine operator was not trained in lockout/tagout procedures prior to operating the machine. This condition exposed the employee to amputation hazards from rotating parts as employees passed by guards to make machine adjustments.

29 CFR 1910.159(c)(10): The minimum vertical clearance of 18 inches between automatic sprinkler systems sprinklers and the material below was not maintained.  On or about November 17, 2016 and at times prior thereto, in the employee training room located at 801 N. Jefferson Street, Springdale, Arkansas, a computer server was stored approximately one inch from the ceiling, exposing employees to fire hazards.
