29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:  The employer did not provide standard railings on the roofs of the buildings, including the Facilities and Special Housing areas to protect the employees from falling about 26 feet to the ground below.  Federal Bureau of Prisons - Atwater Penitentiary was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.23(c)(1), which was contained in OSHA inspection number 1182825, Notice 1,Item 1 and was affirmed as a final order on 11/17/2016, with respect to a workplace located at 1 Federal Way, Atwater, CA 95301.

29 CFR 1910.145(c)(2)(i): Caution signs shall be used only to warn against potential hazards or to caution against unsafe practices.  In the following locations, warning signs were not used:  a) Special Housing Unit Roof: There were no caution signs to warn employees for fall protection or identify designated walking paths. The hatches did not have caution or warning stickers, in one instance where a hatch was measured thirteen inches from the edge of the roof. The roofs have parapet walls varying in heights from five and a half inches to eighteen inches.      b) Health Services Building Roof: Some electrical grounding cables were loosely bounded and could potentially expose employees walking to tripping hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Food Service:  A power strip was placed near the wall on a Pulsafeeder Boiler Controller and used in lieu of fixed wiring to power three metering pumps in the Mechanical Room.
