29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:     a.)    On or about November 7, 2016, the employer did not ensure employees were trained in and familiar with electrical safety related work places before repairing the 480V electrical cord pig tail in the Plasma Cutter area in Plant 2. Employees were required to perform electrical work on 480V and did not have any training on Electrical Safety Related Work Practices.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:     a.)    On or about November 7, 2016, the employer did not ensure employees utilized electrical safety related work practices while repairing a 480V electrical cord pig tail in the Plasma Cutter area in Plant 2. Employees were required to repair the insulation on the 480V electrical cord and were exposed to electric shock and arc flash hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    a.)    On or about November 7, 2016, the employer did not ensure employees were protected while working on a damaged 480V electrical cord. Employees were required to repair the 480V electrical cord pig tail in the Plasma Cutter area in Plant 2 and did not wear the required face, eye, hand, and body protection, exposing them to electrical shock and arc flash hazards.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:    a.)    On or about November 21, 2016, the employer did not certify that periodic audits of the energy control procedure were completed at least annually. Employees are required to perform servicing and maintenance activities on machines including but not limited to, welders, plasma cutters and a roll former, which require energy control procedures that have not been reviewed for deficiencies.
