29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a.) On or about November 7, 2016, the employer did not ensure employees utilized electrical safety related work practices while repairing a 480V electrical cord pig tail in the Plasma Cutter area in Plant 2. Employees were required to repair the insulation on the 480V electrical cord and were exposed to electric shock and arc flash hazards.
