Omega Logging, Inc., 21 Council St., Wheatland, PA , on or about November 21st, 2016 and times prior thereto: Employees worked with and around hazardous substances including, but not limited to, wood dust (Beech, Maple, Oak) during sawing operations of wood, log marking paint, and Schaeffers brand #238 ultra-supreme grease during routine maintenance of mill equipment at the facility. The employer did not develop and implement a written Hazard Communication Program.
