29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) On or about November 14, 2016: on a 2:12 pitch roof one employees was installing anchors for fall protection close to an area where skylights were present and was not protected by guardrail systems, covers, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employee fell approximately 13-feet to ground through a skylight.
