29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: ROOF ABOVE MAIN ENTRANCE: On or about November 22, 2016, an employee was observed to be working at the edge of the roof of the building without railings or any other means of fall protection. The roof was approximately 14 feet above grade.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: 1022 ELM STREET, ROCKY HILL, CT: On or about November 22, 2016, an employee was observed spraying a solution of 50% bleach/50% water on the outside of the building. The employee did not have any eye/face protection to prevent the solution from being sprayed in his face.
