Employees were exposed to hot surfaces from the die and sheet rollers, on machine #20 located in the production area, while removing the die from the equipment.

Haviland Plastic Products Inc. located in Haviland Ohio: On or about November 16, 2016, the employer did not ensure the data plate on the #48 Yale forklift, Model# GLP050VXEVSE084, used to lift a 1300 pound mold from machine #20, was marked to include the extendable boom front end attachment showing the capacity and approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered. Haviland Plastic Products, Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(a)(5), which was contained in OSHA inspection number 1055722, citation number 1, item number 4 and was affirmed as a final order on June 8, 2015, with respect to a workplace located at 119 Main Street, Haviland, Ohio 45851.
