29 CFR 1910.134(c)(2)(i): The employer determined that voluntary respirator use was permissible, but did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"): (a) Haviland Plastic Products Inc., on or about March 6th, 2020, at the location of 119 Main St., Haviland, OH, 45851: The employer provided DuraMask dust masks to employees for use on a voluntary basis without providing employees with the information contained in Appendix D of the respiratory protection standard which covers limitations and precautions for safe use of the respirator.
