29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown was not utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. Modern Plastics Recovery, Inc. located in Haviland, Ohio: On or about November 16, 2016, the employer did not ensure an employee using a long metal rod to unjam bridged plastic in the hopper located in the bin room, turned off the auger inside of the bin prior to performing the operation.
