29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Modern Plastics Recovery, Inc. - Haviland, Ohio: On or about August 28, 2017, the employer did not develop, document, and utilize energy control procedures for authorized employees who service and unjam the cyclone's rotary airlock system located at the shredder. Employees were exposed to amputation injury in that energy control was not utilized while unjamming the machine.

29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures. Modern Plastics Recovery, Inc. - Haviland, Ohio: On or about August 28, 2017, the employer did not ensure authorized employees were retrained on safe application, methods, and means to isolate and control energy when there was a change in job assignment and change in equipment that presents a new hazard. Employees that unclogged plastic from the cyclone's rotary airlock system, located at the shredder, did not receive retraining in lockout/tagout for the machine.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a. Modern Plastics Recovery, Inc. - Haviland, Ohio: On or about August 28, 2017, the employer did not ensure that employees were protected from hazardous energy when unclogging plastic from the cyclone's rotary airlock system located at the shredder. Employees unjamming the machine were exposed to pinch point and amputation injury in that the mechanical energy source was not isolated. Modern Plastics Recovery, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(2), which was contained in OSHA inspection number 1260013, citation number 1, item number 1 and was affirmed as a final order on January 18, 2017, with respect to a workplace located at 113 W. Main St., Haviland, Ohio 45851.
