29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.    Jobsite:  Employees were using a scaffold system that was part ladder jack and part pump jack that had not been erected by a competent person.  This exposed the employees to fall and struck-by hazards.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    Jobsite: Employees were using a pump jack scaffold pole that was not secured by rigid triangular bracing or equivalent at the bottom.  The pump jack assembly was installed backwards and used in conjunction with ladder jack scaffolding exposing them to fall and struck-by hazards.

29 CFR 1926.452(k)(1): Ladder Jack Scaffold Platforms exceeded a height of 20 feet (6.1 m):    Jobsite:  Employees were using a ladder jack scaffold that was approximately twenty-one (21) feet high on one end and approximately twenty-three (23) feet high on the other end exposing employees to fall and struck-by hazards.      Ridge Brothers Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 401082, citation number 1, item number 3-a and was affirmed as a final order on June 22, 2012, with respect to a workplace located at 95 Bridge Street, Beverly, MA 01915.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      Jobsite:  Employees were using a ladder that did not extend more than three (3) above the landing, was not secured at its top, nor was a grasping device provided exposing them to a fall of approximately twenty-one (21) feet to the concrete below.
