29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: On or about November 16, 2016, an employee working in the Raw Material Receiving Area in the Rendering Plant was exposed to trip and fall hazards because there was standing water on the floor of the work area.

29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentally walk shall be guarded by a floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing. On or about November 16, 2016, an employee working in the Raw Material Receiving Area in the Rendering Plant was exposed to trip and fall hazards because the floor opening did not have a floor hole cover or standard railing.
