29 CFR 1926.417(b): Equipment or circuits that were de-energized did not have tags attached at all points where such equipment or circuits could be energized: At the jobsite located at 5615 William Street, Omaha, NE 68106, the employer failed to protect employees from electrical shock hazards and entanglement hazards created by the unexpected energization of a gas fired furnace in that; a tag was not affixed at the circuit breaker where the equipment was de-energized.
