29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: (a) American Residential Services, LLC dba ARS / Rescue Rooter at 1540 Willow St. Strasburg, CO 80136: On 1/28/2020 the employer did not protect employees from injury in that the employer did not ensure that underground utilities were exposed in a safe manner. A water line and an electrical line were severed with a excavator bucket while digging to repair a sewer line. A gas line was nicked with a hand shovel. These conditions exposed employees to electrical shock and fire hazards.
