29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  The employer is failing to protect employees from struck by hazards associated with operating pneumatic tools in the reroofing operations on an existing residential home.  The most recent occurrence of this happened on a job site located at 2401 Sunburst Dr. Papillion, NE where the employer had employees using nail guns rated between 80-120 PSI to install roof shingles on an existing residential home without wearing any eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   The employer is failing to protect employee(s) from falls while working on a roof at 2401 Sunburst Dr. Papillion, NE.  The pitch of the roof is greater than 4:12 and the height to the eave was estimated to be approximately 9 feet 8 inches.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from a fall hazard at 2401 Sunburst Dr. Papillion, NE, in that the ladder used to access the roof was not extended 3 feet above the landing surface
