29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on July 31, 2018 on the jobsite located at 12204 S. 79th St, Omaha, NE 68046.  Eight employees were re-roofing an existing single-story residential home.  The roof has a five in twelve steep sloped pitch at heights greater than six feet above the ground.       Francisco Javier Rios dba Rios Roofing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1193782, citation number 1, item number 2, and was affirmed as a final order on February 1, 2017 with respect to a workplace located at 2401 S. Sunburst Dr, Papillion, NE 68046.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof.  This most recently occurred on July 31, 2018 on the jobsite located at 12204 S. 79th St, Omaha, NE 68046.  Eight employees are using an aluminum extension ladder to enter and exit the roof where the side rails were not extended to at least 3 feet above the landing surface which is about fourteen feet above the ground.       Francisco Javier Rios dba Rios Roofing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1193782, citation number 1, item number 3, and was affirmed as a final order on February 1, 2017 with respect to a workplace located at 2401 S. Sunburst Dr, Papillion, NE 68046.
