29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: On or about November 25, 2016, during the installation of structural framing material (plywood sheeting/decking) at a job site located at 100 N. Ave, Tallmadge, Ohio 44278; the employer did not ensure employees working on the 4th floor installing sheeting/decking on the floor joists were protected from a fall of approximately 20.4 feet by utilizing a fall protection system. The Swiss Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(1) Duty to have fall protection), which was contained in OSHA inspection number 1005118, citation number 01, item number 02 and was affirmed as a final order on 12/18/2014, with respect to a workplace located at 4930 Transportation Dr., Sheffield, Ohio 44035.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about November 25, 2016, during the installation of structural framing material at the commercial project site located at 100 N. Ave, Tallmadge, Ohio 44278 an employee was injured resulting in in patent hospitalization; the employer did not notify OSHA that an employee fell approximately 20.4 feet causing severe injury and hospitalization.
