This most recently occurred on November 28, 2016 on the jobsite located at 22125 Pine Hill Dr, Gretna, NE 68028.  Employees were using a mobile tubular scaffolding system to reach elevated heights where sprayed-on foam insulation needed to be grinded down and smoothed out.  The scaffold was not inspected prior to use and the wheels were left unlocked while being used by the employee.

The first level, where the employee was working, is 6.75 feet above the concrete floor.  None of the four wheels on the mobile scaffold were locked while the employee was working on it.
