29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: The employer is failing to protect employees from fall hazards by ensuring that the mobile scaffolding system was inspected for defects and violations of safety and health requirements prior to allowing an employee to use it. This most recently occurred on November 28, 2016 on the jobsite located at 22125 Pine Hill Dr, Gretna, NE 68028. Employees were using a mobile tubular scaffolding system to reach elevated heights where sprayed-on foam insulation needed to be grinded down and smoothed out. The scaffold was not inspected prior to use and the wheels were left unlocked while being used by the employee.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: The employer is failing to protect employees from fall hazards by ensuring that the mobile scaffolding system being utilized has the wheel locks engaged. This most recently occurred on November 28, 2016 on the jobsite located at 22125 Pine Hill Dr, Gretna, NE 68028. The first level, where the employee was working, is 6.75 feet above the concrete floor. None of the four wheels on the mobile scaffold were locked while the employee was working on it.
