Palletizing, Line 3:  On, or about, December 5, 2016, emergency stop controls were not provided at work stations along the conveyor system.  The only control station was located on the non-working side of the conveyor belt, near the finish end.       B.  Palletizing Line 1:  On, or about, December 6, 2016, emergency stop controls were not provided at the finish end work station.  They only control station was located on the non-working side of the conveyor belt.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    A.  Section A Mezzanine:  On, or about, December 6, 2016, employees were exposed to a fall hazard from the open sided floor of the mezzanine during forktruck deliveries.  The single chain that was provided was not the equivalent of a standard guardrail in that it had more than three inches of deflection and midrail equivalent was not provided.  Also, employees were not provided with any fall protection when the single chain was removed and replaced with the overhead door open.     B.  Side B Packaging Mezzanine:  On, or about, December 6, 2016, the chain used in lieu of guardrails for the material delivery areas was not equivalent to standard guardrails in that the was more than three inches of deflection in the chain and the chain on the right side, facing, was not provided with a midrail equivalent.

At the time of the inspection, there were no active construction operations.  The company did not maintain the exit (e.g., provide a temporary landing outside of the door) and make temporary arrangements for the use of an alternative exit during actual construction operations near the egress route outside of the exit door.
