29 CFR  1910.147(f)(3)(ii)(D):  Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  Company:  On, or about, March 22, 2021, employees who performed sanitation activities had not been provided with locks to affix to group lockout hardware while cleaning and sanitizing production equipment.   Normally, supervisors locked out production equipment at the beginning of the sanitation shift.  Several employees did not have their own locks to affix to the lockout points.

29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Production Floor:  Line 2, Side B, Pasta Technologies Group Bi-Colored Filled Pasta Line:  On, or about, March 19, 2021, employees were exposed to rotating paddles that were located in the front and back sheeter hoppers while transferring pasta dough from the rear conveyor to the front sheeter hopper.  Employees were required to hold a bucket between the rear conveyor discharge and open hopper to collect dough and then dump the bucket into the open front hopper while the machine was in operation.

29 CFR  1910.147(c)(4)(ii):  Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  Production Floor:  Line 2, Side B, Pasta Technologies Group Bi-Colored Filled Pasta Line:  On, or about, March 22, 2021, an employee was severely injured when he was pulled into the top of the front hopper while performing a quality inspection.  The lockout/tagout procedure for the machine did not address cleaning and inspecting the machine for sanitation purposes.  The hopper could not be manually cranked during the inspection process and the machine was routinely inspected while running. Additionally, at the time of the accident, a sanitation employee was still in the process of cleaning the front of the machine after the locks had been removed by a Supervisor.  Carla's Pasta, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.147(c)(4)(i)], which was contained in OSHA Inspection Number 1194248, Citation Number 1, Item Number 6 and was affirmed as a Final Order on March 28, 2017, with respect to a workplace located at South Windsor, Connecticut.

29 CFR  1910.147(f)(1)(ii):  When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2):  Production Floor:  Line 2, Side B, Pasta Technologies Group Bi-Colored Filled Pasta Line:  On, or about, March 22, 2021, an employee was severely injured when he was pulled into the top of the front hopper while performing a quality inspection.  The employer had not implemented procedures to protect employees during quality inspections when machines were released from lockout and turned on to check for any material that may have been missed during the cleaning procedure.  Carla's Pasta, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.147(c)(4)(i)], which was contained in OSHA Inspection Number 1194248, Citation Number 1, Item Number 6 and was affirmed as a Final Order on March 28, 2017, with respect to a workplace located at South Windsor, Connecticut.

The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  Located at Carla's Pasta, Inc., 50 Talbot Lane, South Windsor, Connecticut:  On or about March 22, 2021, the employer failed to electronically submit information from their 2020 OSHA Form 300A or equivalent by March 2, 2021.  a).
