29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard(s) identified: (a)AMTEC Corporation d/b/a Tech Ord @ 47600 180th St., Clear Lake, SD 57226: On and prior to 11/29/2016, the employer did not ensure employees were protected from struck-by hazards as a result of the accidental detonation of the Mark 96 detonator while attempting to heat shrink lead wires to the device. The employer did not adequately assess the hazard and select the proper hand protection. On 11/29/2016, an employee's hand and wrist were injured as a result of an explosive event causing shrapnel to penetrate the gloves the employee was wearing. The condition exposed employee's hands to struck-by hazards in the form shrapnel caused by detonation.
