29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5): 1735 building, warehouse - on or about November 1, 2016, powered industrial truck operators were not provided with adequate initial training as required by paragraph (l), prior to being assigned to operate powered industrial trucks for regular material handling tasks. Adequate truck and workplace related topics and exercises were not completed for permanent and temporary operators. Employees were exposed to struck-by hazards likely to result in serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) 1735 building, warehouse, amazon area desk, relocatable power tap: on or about December 1, 2016, employees were exposed to fire and electrical hazards where an energized relocatable power tap was used for powering multiple electronic devices. Relocatable power tap was not directly connected to a permanently installed branch-circuit receptacle outlet and was connected in series to other extension cord. This equipment is not intended for such use. (b) 1735 building, warehouse, distribution center manager's office desk, Fellowes relocatable power tap: on or about December 1, 2016, employees were exposed to fire and electrical hazards where an energized relocatable power tap was used for powering multiple electronic devices. Relocatable power tap was not directly connected to a permanently installed branch-circuit receptacle outlet and was connected in series to other extension cord. This equipment is not intended for such use. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
