29 CFR 1926.601(b)(14): All vehicles in use were not checked at the beginning of each shift to assure that the following parts, equipment, and accessories were in safe operating condition and free of apparent damage which could cause failure while in use: service brakes, including trailer brake connections; parking system (hand brake); emergency stopping system (brakes); tires; horn; steering mechanism; coupling devices; seat belts; operating controls; and safety devices:    On or about November 16, 2016, at the stokes force main excavation worksite, 670 Panama Street in Hot Springs, Arkansas, the #80 2008 Kobelco SK350LC Excavator being used by employees was not properly inspected prior to its use.  The employer failed to:    (a)  Inspect the cylinder to ensure their is no unexpected release of hydraulic fluids.    (b)  Visually inspect the connection between the excavator bucket and coupler to ensure proper connection prior to material lifting.    (c)  Ensure locking pins were installed prior to material lifting.    Failure to do so exposed the excavator bucket to unexpected detachment and employees to struck-by hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    On or about November 16, 2016, at the stokes force main excavation worksite, 670 Panama Street in Hot Springs, Arkansas, employee in a vertical wall excavation greater than 5 feet depth was not protected while engaged in lubricating and marking PVC sewer pipe.  Employee was exposed to the hazard of being struck by falling excavated materials and/or cave-ins.      The Coakley Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 449853, citation number 1, item number 3 and was affirmed as a final order on September 20, 2012, with respect to a workplace located at I-30 Frontage Road in Bryant, Arkansas.
