29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: (a) Production & Distribution (P & D) Level: Employees were exposed to fall hazards as there were floor holes around the Schur Vertical Conveyors in Cell 1, Cell 2, Cell 3, and Cell 4, and the floor holes were not guarded by either a standard railing with standard toeboard on all exposed sides, or a floor hole cover of standard strength and construction. On or about 11/30/2016, an accident occurred when an employee fell through the floor hole from the second floor P & D Level, to the first floor Distribution Level.
