29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) At 7216 East Chase Parkway, Montgomery, Alabama: On or about December 8, 2016 and at times prior; the employer allowed his employees to work from a flat roof while standing next to a parapet wall which measured 32 inches tall without fall protection exposing them to fall hazard of 20 feet 10 inches to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 7216 East Chase Parkway, Montgomery, Alabama: On or about December 8, 2016 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the roof top and the rails of the ladder did not rise 3 feet or more above the landing area to provide the employees with safe access exposing them to fall hazard of 20 feet 10 inches to the ground below.
