29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a)  Employees who performed maintenance and/or jam removal activities were not required to: (1)  know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy, (2)  turn off and shut down the machines (3)  physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4)  affix lockout or tagout devices to each energy isolating device, (5)  prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.  On or about December 7, 2016 at 471 S Brown Avenue in Peshtigo in the Sawmill, employees removed guards and used their hands inside the Edger and Brewer machines to remove jams, reached inside the Edger machine to lubricate it, and employees performed equipment maintenance on the Head Saw, Trim Saw, Edger and Brewer machines without having knowledge of all power sources and prior to receiving lockout and/or tag out equipment.

All repairs shall be made by authorized personnel.  a)  On or about December 7, 2016 at 471 S Brown Avenue in Peshtigo in the Sawmill area, employees were operating a Komatsu 180 loader with attached forks and with a damaged windshield.  The windshield was smashed in front with full length vertical cracks and smashed on the left side glass with cracks across the width of the glass.

Lumber is placed in front of the running saw at the fence and a single cycle button is pushed to operate the horizontal movement of the blade.  b)  On or about December 8, 2016 at 471 S Brown Avenue in Peshtigo in the Sawmill, employees were standing within 2 ft. of the yellow colored automatic trim saw blade and operating the saw without a two hand control.  Lumber is placed in front of the running saw at the fence and a single cycle pedal is activated to operate the horizontal movement of the blade.

Clement Co. band resaw machine.  The operator  was not protected from ingoing nip point when loading wood into the power feed  rollers and exposed to potential inadvertent contact with uncovered rotating powered gears located the on top and side of feed attachment.

29 CFR 1910.213(i)(1): All portions of the saw blade shall be enclosed or guarded, except for the working portion of the blade between the bottom of the guide rolls and the table. Bandsaw wheels shall be fully encased. The outside periphery of the enclosure shall be solid. The front and back of the band wheels shall be either enclosed by solid material or by wire mesh or perforated metal. Such mesh or perforated metal shall be not less than 0.037 inch (U.S. Gage No. 20), and the openings shall be not greater than three-eighths inch. Solid material used for this purpose shall be of an equivalent strength and firmness. The guard for the portion of the blade between the sliding guide and the upper-saw-wheel guard shall protect the saw blade at the front and outer side. This portion of the guard shall be self-adjusting to raise and lower with the guide. The upper-wheel guard shall be made to conform to the travel of the saw on the wheel.  a)  On or about December 7, 2016 at 471 S Brown Avenue in Peshtigo in the Pallet shop, employees were operating a F.H. Clement Co. band resaw machine with the blade exposed at the front, side and back of the machine.  Employees hand feed boards into the front and walk along the side to the back to catch the product moving out of the machine.

The guard shall be constructed of heavy material, preferably metal, and the edge of the guard shall come to within three-eighths inch of the plane formed by the inside face of the feed roll in contact with the stock being cut.  a)  On or about December 7, 2016 at 471 S Brown Avenue in Peshtigo in the Pallet shop, employees were feeding boards into the F.H. Clement Co. band resaw machine without a guard that would prevent their hands from touching the feed rollers.  The front stationary guard is 7 in. from the front of the rollers with a 5 in.
