29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that a contracted employee under the supervision of a contracted supervisor was exposed to a struck by hazard while operating an abrasive blaster with a dead man control that was not installed according to the manufacturer's Technical Bulletin #5:    a)    Norfolk Naval Shipyard Portsmouth, Building 172: On or about 12/01/216, a contracted employee, under the supervision of a contracted supervisor and the observation by Norfolk Naval Shipyard personnel during a skills evaluation, duct taped a Western Technology electric remote control switch (dead man) P/N 1001 to a blast hose, the duct tape failed to hold, employee lost control of blast hose, causing blast media to strike his left leg and thigh. The dead man switch was not installed onto the blast hose by using hose clamp part no. 3111 or similar.         Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
