29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: (a) Main shop area, On or about December 06 2016, and times prior employees are exposed to a caught in hazard in that there was a rotating shaft on the Take up wheel machine that was extruding approximately four times the diameter of the shaft but turning at 6-7 RPM with a clutch to stop the turning of the shaft. while employees are walking by and could hit the shaft.
