29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Cut and Square Department: On or about September 10, 2019, and at times prior, the employer exposed employees to caught-in hazards in that the rotating draw bar wheel of the modified South Bend Lathe Co. lathe referred to by the employer as the "Stihl grinder" was unguarded as employees inserted and removed workpieces (unfinished flexible shafts) into the machine for grinding. The workpiece extends out from this shaft and rotates with it.  b) Cut and Square Department: On or about September 13, 2019, the employer exposed employees to struck-by and burn hazards in that the Cut and Square #2 machine was unguarded at the point where sparks are created due to the machine cutting metal shafts to specified lengths. Employees work at and around this machine in the production of flexible shafts.
