29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed: (a) The employer did not ensure that employees who worked on and around the Delta X Radial Arm Saw were protected from the blade, exposing employees to cutting hazards and amputation injury.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) Employees working on and around the de-palletizer were exposed to unguarded and energized 240 Volt AC electrical wires. The electrical wires providing power to the de-palletizer were not enclosed within the electrical box connection point, exposing employees to an electrical shock hazard.
