29 CFR 1910.178(l)(3)(ii)(I): The operator training program did not address other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation. Employees engaged in material handling activities using the Drexel swing mast forklift were exposed to struck-by and caught-between hazards in that the forklift training program did not address the workplace specific hazards associated with the use of swing mast forklifts. The struck-by and caught-between hazard is created when the Drexel swing mast fork truck, with drivers compartment offset to the left side of the truck, passes by the protection post, door frame, and the inside of the trailer.
