29 CFR 1910.29(b)(1): (1) The top edge height of top rails, or equivalent guardrail system members, are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11). Employees were exposed to fall hazards. Employees working at the controls of the Master Hydraulic Embossing machine in the RPS department were exposed to a six feet fall hazard into the pit next to the Master Hydraulic Embossing machine. The pit guardrails were not at least 42 inches in height.

1910.30(a)(1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. Employees engaged in production activities at the controls of the Master Hydraulic Embossing machine in the RPS department were exposed to a six feet fall hazard into a pit next to the machine. The employer did not provide fall protection training to the employees.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees were exposed to caught-in hazards. Employees working at the Master Hydraulic Embossing machine in the RPS department were exposed to caught in hazards in that adequate guarding was not provided to prevent employees from reaching the ingoing nip points of the machine rollers.
