29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface. On or about 12/08/2017 at the Acura of Berlin Roofing Project, employees were installing a new roof and were working at and near the warning line system that was not a minimum of 34 inches.

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: On or about 12/08/2016 at the Acura of Berlin Roofing Project, employees were working at the unguarded edges of the roof deck and the safety monitor was doing other work that took his attention away from his monitor duties.
