29 CFR 1910.268(d)(1): Before work was begun in the vicinity of vehicular or pedestrian traffic which could endanger employees, warning signs, flags, barriers, and/or other traffic control devices were not placed conspicuously to alert and channel approaching traffic: On or about December 9, 2016, and in times prior thereto, technicians installing fiber serving terminals did not use barriers or other traffic control devices to protect their path of travel that was shared by vehicles.

29 CFR 1910.268(e): Personal protective equipment, protective devices, and special tools needed for the work of employees were not provided and/or the employer did not ensure that they were used by employees: On or about December 9, 2016, and in times prior thereto, technicians installing fiber serving terminals did not wear a high visibility vest to ensure dump truck drivers and spotters and/or other vehicles could see them when dump trucks and vehicles were operating in close proximity to them.
