29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  The employer did not establish an energy control program consisting of adequate energy control procedures, employee training, and periodic inspections when employees were performing servicing and maintenance work on various equipment and machines including agricultural tractors and combines on a daily basis.

29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  The employer did not develop and implement written energy control procedures for employees participating in group lockout tagout when performing servicing and maintenance work on various equipment and machines including agricultural tractors and combines.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about December 3, 2016, an employee was exposed to the caught-in hazards of an exposed rotating feed auger while cleaning a John Deere 9560 Combine located at N6701 Harris Court Westfield, WI.  The employer did not ensure employees implemented energy control application steps prior to them engaging in cleaning activities of the combine.  An employee energized a feed auger in the combine hopper while another employee attempted to clean it with compressed air while the auger was running.  The machine was not deenergized prior to the employee's engaging in the servicing/maintenance activity.    As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.242(b): Compressed air used for cleaning. Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment:    On or about December 3, 2016, employees utilized a homemade steel air wand that was not equipped with an air pressure reducer to clean combines during inspections and servicing at the N6701 Harris Court Westfield, WI 53964 location.
