29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: a) In the Pallet Shop, at the Woodmizer MUH head band saw, on or about December 12, 2016, accumulations of scrap wood, debris, and saw dust created fire, slip, trip, and fall hazards. b) In the Pallet Shop, at the Newman KM-16 head saw, on or about December 12, 2016, accumulations of scrap wood, debris, and saw dust, created fire, slip, trip, and fall hazards. The Lamb Brothers Lumber Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(a)(2), which was contained in OSHA inspection number 922614, citation number 1, item number 1b and was affirmed as a final order on 2/11/2014, with respect to a workplace located at 102 Industrial Street, Wrens, GA 30833.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) Throughout the Pallet shop and surrounding yard, on or about December 12, 2016 and at times prior to, employees were exposed to a hazard of being struck by the Hyster #50 forklift which had a cracked windshield. b) Throughout the Pallet shop and surrounding yard, on or about December 12, 2016 and at times prior to, employees were exposed to a hazard of being struck by the Hyster #50 forklift that did not have a functioning head lights for use during the early morning shift while working inside. c) Throughout the Pallet shop and surrounding yard, on or about December 12, 2016 and at times prior to, employees were exposed to a hazard of being struck by the Hyster #50 forklift which had a power steering pump leak. d) Throughout the Pallet shop and surrounding yard, on or about December 12, 2016 and at times prior to, employees operated a Cat forklift #928G that did not have a functioning left rear light for use during the early morning shift while working inside. e) Throughout the Pallet shop and surrounding yard, on or about December 12, 2016 and at times prior to, employees operated a Cat forklift #928G that did not have a functioning back-up alarm. f) Throughout the Pallet shop and surrounding yard, on or about December 12, 2016 and at times prior to, employees were exposed to a hazard of being struck by the Yale Veracitor #50VX forklift, which had the hydraulic line leaking. g) Throughout the Pallet shop and surrounding yard, on or about December 12, 2016 and at times prior to, employees operated a Yale Veracitor #50VX forklift, which had its windshield knocked out.
