29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about November 30, 2016, and times prior thereto, employees were observed working on a residential roof with a pitch of 2:12 at heights greater 6 feet without the use of fall protection, exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: The De La Rosa Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1066922, citation number 1, item number 2 and was affirmed as a final order on 07/01/2015, with respect to a workplace located at 102 Country Club Parkway, Maumelle, Arkansas 72113. On or about November 30, 2016, and times prior thereto, at the jobsite, employees were ascending and descending from the rooftop on an aluminum extension ladder which did not extend at least three feet above the rooftop landing, exposing employees to fall hazards greater than 6 ft. to the ground below.
