29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. One Beacon Drive, Palmer, MA: On or about December 13, 2016 employees were observed installing fascia trim on a residential roof with unprotected sides and edges more than 17 feet above the next lower level without being protected from falling by the use of guard rail systems, safety net systems or personal fall arrest systems. LMP Construction Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(1), which was contained in OSHA inspection number 921757 , citation number 1, item number 1 and was affirmed as a final order on September 9, 2013 , with respect to a workplace located at 560 Mineral Spring Ave Pawtucket, RI 02860. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.503(b)(1): The employer did not verify fall protection training by preparing a written certification record. 1 Beacon Street, Palmer, MA: On or about December 13, 2016 employer had not verified fall protection training by preparing a written certification record fro employees that were exposed to fall hazards while replacing fascia trim boards on residential apartment buildings with fall greater than 17 feet. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
